Southern New Hampshire Health simplifies access to primary care providers
NASHUA – Southern New Hampshire Health recently launched a new program to help individuals more easily connect with a primary care provider.
Southern New Hampshire HealthMatch reduces wait times and removes barriers to accessing health care in our community.
“Timely access to primary care is a concern for many people in our region,” said Steven Schapp, executive director of Access and Engagement Systems at Southern New Hampshire Health. “HealthMatch is one way we are working to streamline the process and help people connect to care when and where they need it.”
Southern New Hampshire HealthMatch connects residents to a network of trusted primary care providers within Foundation Medical Partners, based on factors such as location, availability and patient preferences. Through a single number, 603-880-3279, community members can speak directly with a care coordinator to identify an appropriate local provider and set up an appointment, sometimes even the next day.
The service comes at a time when primary care access continues to be a challenge across New England, with many practices at or near capacity. Southern New Hampshire HealthMatch addresses this issue by leveraging the reach of Foundation Medical Partners’ provider network, which includes practices in Amherst, Hudson, Merrimack, Milford, Nashua, Pelham and Pepperell, Mass.
Primary care providers within the network offer a range of services, including preventive care, chronic disease management, treatment for acute conditions, referrals to specialists, and coordination of care when needed. Southern New Hampshire HealthMatch is available to new patients, individuals seeking a new provider, or those reestablishing care after time away from the healthcare system.
